Sharing Your Blood Pressure Log With Your Clinician

Measuring and logging guide ยท Last reviewed November 2, 2026

A clear log is most helpful when it reaches your clinician in a form they can read quickly. Here are common ways to share it.

In person

  • Bring the averages, highest and lowest readings, and any missing sessions.
  • Bring the monitor too, if your clinician wants to compare it with their equipment.

Patient portal or email

Many clinics let you send a message or upload a document through a patient portal. Ask what format they prefer, and avoid sending health details through ordinary email unless your clinic says it is acceptable.

Telehealth

Have your summary open on screen or written down so you can read the numbers aloud. Say how the readings were taken: arm, cuff, time of day.

Keep a copy

Keep your own copy of everything you send, in case a message is not received.

Ask what they want

Some clinicians want every reading, others only the summary. A short call to the office saves a wasted appointment.
